Requirements for Application in Metro Manila
- Clearance from the Traffic Management Group (TMG), Philippine National Police.
- Clearance from the Metropolitan Manila Development Authority (MMDA).
- Submission of an Affidavit of Loss in the prescribed format.
- Certified photocopies of the Official Receipt (OR) and Certificate of Registration (CR) by the Record Officer.
- Clearance from Law Enforcement Service (C.O.).
- Motor Vehicle Inspection Report from the Motor Vehicle Inspection System (MVIS).
- Proof of payment for the cost of the duplicate plates.
Requirements for Application Outside Metro Manila
- Clearance from the Traffic Management Group (TMG), Philippine National Police.
- Clearance from the Local Government Units (Municipality or City Hall).
- Submission of an Affidavit of Loss in the prescribed format.
- Certified photocopies of the OR and CR by the Record Officer.
- Motor Vehicle Inspection Report from MVIS or from the Regional Office where MVIS is not available.
- Clearance from the Law Enforcement Service and/or the Regional Office Operations Division.
- Proof of payment for the cost of the duplicate plates.
Processing and Documentation Procedures
- Only Requisition and Issue Vouchers for duplicate plates accompanied by certified true copies of all supporting documents are to be forwarded to the Central Office for processing.
- All original documents are retained at the respective Regional Office to ensure safekeeping and proper control.
