    • Uncontrolled loss of 150 MW or 5% of system demand for over 15 minutes due to a single incident.
    • Load shedding of 100 MW or 5% of system demand under emergency operations.
    • System-wide voltage fluctuations exceeding 5%.
    • Actual or attempted physical or cyber attacks impacting system adequacy or reliability.
    • Complete operational failure or shutdown of transmission system.
    • Weather or natural calamities affecting system adequacy.
    • Contingency Reserve falling below largest generating unit capacity or zero reserve.
    • Imminent overloading of transmission lines or equipment.

Recipients and Notification Methods of Significant Incident Reports

  • Key government and industry officials to be notified include DOE Secretary and Undersecretary for Electric Power, GMC Executive Director, TransCo and NGCP Presidents, NGCP technical officers, PEMC VP for Market Operations, ERC consumer affairs and regulatory services, NGCP head for operations and maintenance, and affected grid users.
  • Notifications may be made via text message, email, fax, or telephone.

Reporting Schedule and Procedures

  • Initial Report: To be submitted within 60 minutes of disruption using standardized forms (Annex B-1 and B-2), with as much information as available.
    • Informal notifications (texts or calls) allowed to fulfill mandatory reporting when operations are affected.
  • Follow-up Reports/Updates: Submitted as needed to provide additional information.
  • Final Report: Due within 2 weeks of incident, including a detailed narrative (Annex B-3), findings, system impact analysis, causes, corrective actions, and recommendations.
  • Preferred submission methods are electronic, via email or fax.

Special Investigations

  • Optional special investigations may be initiated by the System Operator, Grid Owner, or Users as stipulated under PGC Section 6.7.2.
  • Such investigations may involve inquiries to industry participants or other entities for technical information about incidents.

Content Requirements for Incident Reports

  • Schedule I (Initial Report) includes:
    • Reporter information (name, position, organization, contact details).
    • Incident details: date/time, affected areas, demand impact, nature and probable causes of incident, actions taken.
  • Schedule II (Final Report), due within 2 weeks, must include:
    • Event summary.
    • Detailed findings and analysis.
    • Corrective and preventive measures.
    • Long-term recommendations.
