Forms for Record-Keeping

  • DDB Form No. 13-73: Used by pharmacists in hospitals and similar institutions to keep records of dangerous drugs dispensed to in-patient wards.
  • DDB Form No. 5-72: Used by all other pharmacists not in hospitals for compliance with record-keeping requirements under Section 25(a).

Reporting Requirements for Hospital Pharmacists

  • Hospital pharmacists must complete records on Form No. 14-73 covering dangerous drugs dispensed in in-patient wards.
  • These reports are to be submitted quarterly within 15 days after the end of each quarter.
  • The City or Municipal Health Officer receives the reports first and has 15 days to forward them to the Dangerous Drugs Board.

Reporting Requirements for Non-Hospital Pharmacists

  • Pharmacists outside hospitals must forward certified true copies of records covering each quarter (3 calendar months) using Form No. 5-72.
  • These are submitted to the City or Municipal Health Officer within 15 days after each quarter ends.
  • The Health Officer then forwards these records to the Dangerous Drugs Board within another 15 days.

Special Dispensing Procedures for Hospitals

  • Due to the unique procedures in hospitals, prescriptions for dangerous drugs may be issued immediately after each vial is consumed.
  • All records of the disposition of each vial must be accompanied by properly filled DDB Form No. 13-73.

Effectivity

  • The regulation took effect immediately upon adoption on February 21, 1973.
