The regulation prescribes the records and reports required of pharmacists on the sales, purchases, acquisitions, and deliveries of dangerous drugs, pursuant to the Dangerous Drugs Board's powers under Section 36(a) of R.A. 6425, as amended.

Pharmacists in hospitals must use DDB Form No. 13-73 to keep records of dangerous drugs dispensed in the in-patient wards.

All other pharmacists must use DDB Form No. 5-72 when complying with the record-keeping requirements of Section 25(a) of R.A. 6425.

Hospital pharmacists must submit quarterly reports using Form No. 14-73 to the City or Municipal Health Officer within 15 days following each quarter's end.

The City or Municipal Health Officer must forward these reports within 15 days from receipt thereof to the Dangerous Drugs Board.

They must submit certified true copies of records covering a period of three calendar months (quarterly) to the City or Municipal Health Officer.

Due to the peculiar procedures, the required prescription form for dangerous drugs in hospitals may be issued immediately after each vial is consumed, provided each disposition record is accompanied by properly filled forms conforming to DDB Form No. 13-73.

These regulations are prescribed under Section 36(a) of R.A. 6425, as amended, and relate to compliance with Section 25(a) of the same law.
