Purpose and coverage
- Sponsored Program members must use the PhilHealth Identification Card (ID) in lieu of the Family Health Card starting October 1, 2010.
- The new PhilHealth ID serves as proof of membership registration for sponsored members.
- The card also covers certification for the cardholder and his legal dependents to PhilHealth benefits.
What the new PhilHealth ID must show
- The new PhilHealth ID is rendered in the same design as the PhilHealth Number Card earlier issued to members in the employed Sector and Individually Paying Program.
- The PhilHealth ID must bear the following details:
- Permanent PhilHealth Identification Number (PIN)
- Complete name
- Signature
- 1a x 1a photograph
- Effectivity Date pertaining to the validity period of coverage
- The front of the card must show:
- The PhilHealth corporate signature at the upper left-hand portion above the 1a x 1a photograph box
- A magnified PhilHealth icon rendered in watermark
- Yellow and green fine, curved lines as background
- The back of the card must include:
- Certification as to identity and entitlement of the cardholder and his legal dependents to PhilHealth benefits
- Signature of the PhilHealth President and CEO
- Official seal of the Republic of the Philippines rendered as watermark
Validity, honoring older Family Health Cards
- Previously-issued Family Health Cards remain honored as proof of eligibility for sponsored members and their qualified legal dependents.
- Eligibility using previously-issued Family Health Cards is allowed only when availment occurs within the validity period indicated in the card.
