The new PhilHealth Identification Card serves as the proof of membership registration for members under the Sponsored Program, replacing the Family Health Card starting October 1, 2010.
Members under the Sponsored Program are required to use the new PhilHealth Identification Card.
The front side includes the PhilHealth corporate signature at the upper left, a place for the member's 1x1 photograph, a magnified watermark of the PhilHealth icon, and a background of yellow and green fine curved lines.
The card contains the Permanent PhilHealth Identification Number (PIN), complete name of the member, signature of the member, 1x1 photograph, and the effectivity date pertaining to the validity period of coverage.
The back of the card has certification as to the identity and entitlement of the cardholder and legal dependents, the signature of the PhilHealth President and CEO, and the official seal of the Republic of the Philippines rendered as a watermark.
Dr. Rey B. Aquino, President and CEO of PhilHealth, signed the circular.
The effectivity date indicates the validity period of the PhilHealth coverage granted to the sponsored member.
The official text and design of the new PhilHealth ID card are available at the Office of the National Administrative Register, U.P. Law Complex, Diliman, Quezon City.
Sponsored Members will use the new PhilHealth ID card designed like that issued to members in the Employed Sector and Individually Paying Program, replacing their former Family Health Card.
