Structure and Interpretation of the New Accreditation Number

Issuance and Validity of the New Accreditation ID Cards

  • New accreditation ID cards are issued:
    • Upon initial accreditation
    • Upon re-accreditation
    • Upon upgrading of accreditation
    • During renewal of accreditation
  • The validity period for each issued ID card is three (3) years from the date stated.

Upgrading of Accreditation Procedure

  • Physicians applying for upgrading of accreditation before their current accreditation expires must:
    • Submit all required documentary requirements.
    • Pay an accreditation fee of Php 1,500.
  • A new ID card is subsequently issued to the physician.
  • The new accreditation validity is three (3) years from issuance.

Information Reflected in the Accreditation ID Card

  • Application Status and Validity:
    • Initial Accreditation, Re-Accreditation, and Upgrading: Valid for three years from application stamp to day before birthdate three years later.
    • Renewal of Accreditation: Valid for three years from birthdate to day before birthdate three years later.
  • Remarks on Application:
    • Applicants can apply anytime for initial or upgrading of accreditation.
    • Applicants with a gap of one day or more in accreditation validity are advised to apply promptly.
    • Renewal applications must be filed within four months before accreditation expiry.

Replacement of Lost or Damaged Accreditation ID Cards

  • Procedures to request replacement include:
    1. Submission of an affidavit of loss (where applicable).
    2. Presentation of two (2) 1x1 ID pictures.
    3. Payment of a replacement fee amounting to Php 150.
  • Replacement requests can be made at PhilHealth Regional Offices, Service Offices, or the Central Office Accreditation Department.
