Structure and Interpretation of the New Accreditation Number
- The first four (4) digits of the accreditation number have specific interpretations as detailed in "ANNEX A."

Issuance and Validity of the New Accreditation ID Cards
- New accreditation ID cards are issued:
- Upon initial accreditation
- Upon re-accreditation
- Upon upgrading of accreditation
- During renewal of accreditation
- The validity period for each issued ID card is three (3) years from the date stated.
Upgrading of Accreditation Procedure
- Physicians applying for upgrading of accreditation before their current accreditation expires must:
- Submit all required documentary requirements.
- Pay an accreditation fee of Php 1,500.
- A new ID card is subsequently issued to the physician.
- The new accreditation validity is three (3) years from issuance.
Information Reflected in the Accreditation ID Card
- Application Status and Validity:
- Initial Accreditation, Re-Accreditation, and Upgrading: Valid for three years from application stamp to day before birthdate three years later.
- Renewal of Accreditation: Valid for three years from birthdate to day before birthdate three years later.
- Remarks on Application:
- Applicants can apply anytime for initial or upgrading of accreditation.
- Applicants with a gap of one day or more in accreditation validity are advised to apply promptly.
- Renewal applications must be filed within four months before accreditation expiry.
Replacement of Lost or Damaged Accreditation ID Cards
- Procedures to request replacement include:
- Submission of an affidavit of loss (where applicable).
- Presentation of two (2) 1x1 ID pictures.
- Payment of a replacement fee amounting to Php 150.
- Replacement requests can be made at PhilHealth Regional Offices, Service Offices, or the Central Office Accreditation Department.
