The new accreditation number convention is composed of twelve (12) numerical characters.
The new twelve-digit accreditation number convention was required to be used starting January 1, 2006.
The new accreditation ID cards are released once every three (3) years after initial accreditation, re-accreditation, upgrading of accreditation, and during renewal of accreditation.
The accreditation fee required for upgrading of accreditation is one thousand five hundred pesos (P1,500).
The physician must submit all documentary requirements as required for the upgrade application and pay the accreditation fee.
The applicant must apply during the first month of the four months before the expiration of the accreditation.
The requirements are: 1) Affidavit of loss, as applicable, 2) two (2) pieces of 1 x 1 ID picture, and 3) a replacement fee of one hundred and fifty pesos (Php 150.00).
'Application Status' refers to whether the accreditation is Initial Accreditation, Re-Accreditation, Upgrading of Accreditation, or Renewal of Accreditation. The validity of accreditation is three years, either from the date the application was stamped complete for initial/re-accreditation/upgrading or from the birthdate for renewal.
