Identification of Vital Personnel Documents
- The CSC specifies three documents where the blood type must be indicated:
- Employee Identification (ID) Card issued by the agency.
- Personnel Data Sheet (PDS), also known as CSC Form 212.
- Medical Certificate Form, CSC Form No. 211.
Revision of Forms to Include Blood Type
- CSC Resolution No. 98-2113 officially revises the PDS and Medical Certificate forms by adding a provision to indicate the blood type of employees.
Agency Responsibilities and Implementation Timeline
- Heads of government agencies are instructed to integrate blood typing into their respective employee health care programs.
- Agencies are required to update the PDS of all existing employees to include the blood type.
- Employee ID cards must also be renewed to incorporate blood type information.
- The deadline for full compliance with these requirements is set for the end of 1998.

- It was signed by Corazon Alma G. De Leon, Chairman of the Civil Service Commission, thereby carrying full administrative authority.
