Law
Csc Memorandum Circular No. 35
Decision Date
Aug 18, 1998
The Civil Service Commission mandates all government agencies to include employees' blood types in vital personnel documents to enhance health care and support the National Voluntary Blood Service Program.

Q&A (CSC MEMORANDUM CIRCULAR NO. 35)

The inclusion of blood type in employee records supports the National Voluntary Blood Service Program of the Department of Health.

The employee's blood type must be indicated in the Employee Identification Card, Personnel Data Sheet (PDS) or CSC Form 212, and Medical Certificate Form or CSC Form No. 211.

The CSC Resolution number is 98-2113.

Heads of government agencies must include blood typing in their health care programs for employees.

By the end of 1998, all agencies should have updated the PDS and renewed employee ID cards to contain the blood type of employees.

Including blood type information is important for medical emergencies and supports the voluntary blood service program for efficient blood donation and transfusion.

CSC Form 212 (Personnel Data Sheet) and CSC Form No. 211 (Medical Certificate Form) are mentioned.

The directive applies to all existing government employees, requiring their records and ID cards to be updated with their blood type.
