    • CDA Central Office (Board, Legal, Supervision Unit, Planning, MIS): View, correct (with permission), analyze, report generation, and system maintenance.
  • Creation and Control of Access Code: Managed by CAPRIS Administrator upon approval.
  • Authorization: Users must be authorized and familiar with the system before access.
  • Prescribed Form: Uniform use of the prescribed CAPR Form downloadable from CDA website; incorrect forms to be immediately corrected.

General Policies

  • Accountable Persons: Board Chairperson and General Manager are liable for accuracy and truthfulness of data.
  • User Manual: Developed by MIS and Supervision & Examination Unit.
  • Additional Information Requirements: Identified by Board of Administrators and Planning Division in consultation with Management Committee.
  • Information Dissemination: Managed by Authority through Extension Offices.
  • Correction of Data: Editing of submitted data only allowed after written permission and validation; corrections executed by Supervision and Examination Unit with Legal Department approval.
  • Information and Report Generation: Responsibility of Planning Division.
  • System Error Reporting: Users must document and report system errors promptly for maintenance and improvements.

Miscellaneous Provisions

  • User Training: Mandatory orientation and training for all identified CAPRIS users.
  • Administrative Sanctions: Unauthorized access, transfer, or misuse resulting in loss/damage of system or data will be penalized under applicable laws.

Effectivity

  • Effective immediately upon Board of Administrators approval.
  • Approved via BOA Resolution No. 496, s-2014 dated August 11, 2014.
