Document Requirements for Replacement Plates

  • Certified xerox copy of Registration Certificate (CR) and Official Receipt (OR) must be submitted.
  • For for-hire vehicles, LTFRB (Land Transportation Franchising and Regulatory Board) decisions granting provisional authority of the Certificate of Public Convenience (CPC) must be presented.
  • Confirmation or Verification issued by LTFRB Regional Office is required for for-hire vehicles.
  • Pair of plates to be surrendered must be presented.
  • A Motor Vehicle Inspection Report (MVIR) is necessary.
  • Certification from the issuing DO/EO confirming no prior application for replacement plates.

Document Requirements for Duplicate OR/CR

  • Certified xerox copy of Official Receipt (OR) and Certificate of Registration (CR) must be provided.
  • Affidavit of Loss executed by the registered owner.
  • Clearance from HPG/PNP authority is necessary.
  • Two (2) valid IDs of the registered owner and two (2) valid IDs of the authorized representative are required.
  • A Motor Vehicle Inspection Report (MVIR) must be submitted.
  • Request for confirmation from the issuing DO/EO is required.

Document Requirements for Replacement Stickers

  • Certified xerox copy of Official Receipt (OR) or Certificate of Registration (CR) must be submitted.
  • Affidavit of Loss executed by the registered owner.
  • Clearance from HPG/PNP authority is mandatory.
  • Two (2) valid IDs of the registered owner and two (2) valid IDs of the authorized representative are required.
  • Submission of Motor Vehicle Inspection Report (MVIR) is needed.
  • Affidavit of undertaking that exempts the Land Transportation Office (LTO) personnel from liability for any third-party claims.
