To require uniform documentary requirements for the application of duplicate/replacement plates, duplicate OR/CR, and replacement stickers, primarily for security and protection of both vehicle owners and issuing officials, in response to carnapping cases.
An Affidavit of Loss executed by the registered owner.
Clearance from HPG/PNP.
Authority of the registered owner, plus two (2) valid IDs for both the registered owner and the authorized representative.
A Motor Vehicle Inspection Report (MVIR) is required.
It requires Certification from the issuing DO/EO that no application for duplicate was effected.
Certified xerox copy of the Registration Certificate and certified xerox copy of the Official Receipt.
A pair of plates to be surrendered.
LTFRB Decisions granting the provisional authority of CPC (if for-hire) and Franchise Confirmation/Verification issued by LTFRB in the Region (if for-hire).
Certification from the issuing DO/EO that no application for replacement was effected.
An Affidavit of Loss executed by the registered owner.
Clearance from HPG/PNP.
A Request for Confirmation from the issuing DO/EO.
Certified Xerox copy of Official Receipt (OR)/Certificate of Registration (CR).
Certified Xerox copy of Official Receipt/Certificate of Registration (CR).
An Affidavit of undertaking that LTO personnel shall not be held liable for any third party claim.
Authority of the registered owner; two (2) valid IDs of the registered owner; and two (2) valid IDs of the authorized representative.
Previous instructions/orders/memoranda in conflict herewith are deemed amended/superseded accordingly.
