Color Codes for Application Types

  • Specific colors assigned for each type of application form to streamline document management:
    • New Certificate of Public Convenience (CPC) – LO1 Orange
    • Extension of Validity of CPC – LO2 Dark Blue
    • Sale and Transfer of CPC – LO3 Maroon
    • Amendment of Line – LO4 Mint Green
    • Consolidation of Case Numbers – LO5 Gray
    • Change of Party Applicant – LO6 Black
    • Conversion – LO7 Red
    • Fare Increase/Decrease – LO8 Blue Green
    • Dropping and Substitution of Unit – AO1 Lemon Yellow
    • Dropping of Unit – AO2 Dark Brown
    • Special Permit – AO3 Light Blue
    • Change of Engine – AO4 Dark Green
    • Correction of Entry – AO5 Baby Pink
    • Adoption of Color Scheme or Trade Name – AO6 Golden Yellow
    • Extension of Time to Register – AO7 Royal Blue
    • Installation of Advertisements – AO8 Lavender
    • Re-Registration of Unit – AO9 White

Color Codes for Service Types

  • Different color-coded forms adopted for checklists based on service category:
    • Public Utility Bus (PUB) Service – Blue
    • Tourist Highway (TH) Service – Yellow
    • Public Utility Jeepney (PUJ) Service – Pink
    • Taxi Service – White
    • Asian Utility Vehicle (AUV) Service – Green
    • Special Modes of Service – Lavender

Document Submission Requirements

  • All documents must be securely attached to the designated color-coded folder.
  • Attachments should be clearly marked and tabbed, aligned with numbered checklist requirements.
  • Ensures systematic review and verification by LTFRB personnel.

Supersession and Effectivity

  • The memorandum circular overrides any inconsistent previous issuances.
  • Effective starting 01 May 2005.
  • Filing of three copies required with the UP Law Center in compliance with Presidential Memorandum Circular No. 11 (09 Oct 1992).
