  • Sale and Transfer of CPC must use form LO3 with a Maroon folder.
  • Amendment of Line must use form LO4 with a Mint Green folder.
  • Consolidation of Case Numbers must use form LO5 with a Gray folder.
  • Change of Party Applicant must use form LO6 with a Black folder.
  • Conversion must use form LO7 with a Red folder.
  • Fare Increase/Decrease must use form LO8 with a Blue Green folder.
  • Dropping and Substitution of Unit must use form AO1 with a Lemon Yellow folder.
  • Dropping of Unit must use form AO2 with a Dark Brown folder.
  • Special Permit must use form AO3 with a Light Blue folder.
  • Change of Engine must use form AO4 with a Dark Green folder.
  • Correction of Entry must use form AO5 with a Baby Pink folder.
  • Adoption of Color Scheme or Trade Name must use form AO6 with a Golden Yellow folder.
  • Extension of Time to Register must use form AO7 with a Royal Blue folder.
  • Installation of Advertisements must use form AO8 with a Lavender folder.
  • Re-Registration of Unit must use form AO9 with a White folder.
  • Registration-in-Lieu of Authorized Unit must use form AO10 with a Fuschia folder.
  • Interchange of Units must use form AO11 with a Violet folder.
  • Upgrading/Downgrading must use form AO12 with a Beige folder.

Board-provided checklist formats by service type

  • The Board and its Regional Offices must provide applicants a color-coded Checklist of Requirements for different types of service.
  • PUB Service must use a color-coded checklist form Blue.
  • TH Service must use a color-coded checklist form Yellow.
  • PUJ Service must use a color-coded checklist form Pink.
  • Taxi Service must use a color-coded checklist form White.
  • AUV Service must use a color-coded checklist form Green.
  • Special Modes of Service must use a color-coded checklist form Lavender.

Fastening, attachment marking, and tabbing rules

  • Operators must fasten all documents to the folder.
  • Operators must ensure that all attachments are properly marked and tabbed.
  • Attachment marking and tabbing must correspond to the numbers on the Checklist/Requirement Form.

Effectivity and supersession

  • The circular takes effect on 01 May 2005 after the filing of three (3) copies with the UP Law Center.
  • The circular supersedes any and all issuances inconsistent with its requirements.
