Scope and Functionality of CBCI as a Collecting Agent
- CBCI is authorized to accept premium payments exclusively from IPMs who possess a valid PhilHealth Identification Number (PIN).
- Upon receipt of payment, CBCI must issue a duly validated CBCI Payment Form to the payer.
Contents and Validation Requirements of the CBCI Payment Form
- The CBCI Payment Form must contain specific information as stipulated under Article III item 1.a of the CRA:
- Payer's Name
- PhilHealth Identification Number (PIN)
- Full Name of the Member
- Applicable Period of the premium payment
- Member Type
- Date of Payment
- Amount Paid
- The form must be duly validated by CBCI to be considered official proof.
Legal Recognition of the CBCI Payment Form
- The duly validated CBCI Payment Form bearing all required information shall be recognized as valid proof of premium payment under the National Health Insurance Program (NHIP).
- This recognition facilitates members' proof of compliance with premium payment requirements.
