At the premises of the Insurance Commission Building.

Two (2) identical 1x1 head photographs, recently taken.

An identification card/papers.

An examination fee of P20.00 and a walk-in fee of P30.00.

P50.00 total (P20.00 examination fee + P30.00 walk-in fee).
