The main purpose is to establish a National Policy on the Unified Registry Systems for Chronic Non-Communicable Diseases, Injury Related Cases, Persons with Disabilities, and Violence Against Women and Children Registry System to ensure standardized data collection, reporting, and use for public health planning and policy-making.
The prioritized diseases include chronic non-communicable diseases such as cardiovascular diseases, cancers, chronic respiratory diseases, diabetes, injuries, violence against women and children, and disabilities.
All government and private clinics and hospitals, Centers for Health Development Offices, Provincial/District/City/Municipality Health Offices, medical professional societies, and associations are required to submit reportable cases to the DOH Information Management Service.
Chronic non-communicable diseases and disabilities must be reported monthly, while injuries and violence must be reported daily.
A 'Reportable Case' refers to a diagnosed or confirmed case of a chronic non-communicable disease, injury, violence or disability.
The Order mandates securing the confidentiality, security, and integrity of data at all times. Users are given one account per facility, data access is restricted, personal details are confidential, and unauthorized disclosure is punishable by law.
Failure to comply may constitute a violation subject to penalties as provided under Administrative Order No. 2011-0020 and Administrative Order No. 2007-0022 regarding violations under the One-Stop Shop Licensure System for Hospitals.
The NCDPC manages overall implementation, formulates guidelines, validates data quality, publishes reports, issues sanctions including suspensions and terminations of user accounts, and coordinates with other agencies.
Submitted data cannot be edited directly. To request edits, the reporting health facility must fill an Incident Report (Annex 9.0) signed by the head of the facility, submit it to NCDPC, and wait for confirmation of changes.
The Order applies to all Department of Health Central Office units, Centers for Health Development Offices, Provincial/District/City/Municipality Health Offices, government and private clinics and hospitals, and medical professional societies and associations.
