Q&A (BOC MEMORANDUM)

The first Prison Guard at the scene must immediately conduct an initial investigation, prepare a Spot Report, and submit it to the Investigation Section within three (3) hours of the occurrence. He must also furnish copies to the Commander of the Guard on duty and the Superintendent of NBP, who in turn must immediately inform the Director of Corrections.

The Spot Report must be submitted within three (3) hours from the occurrence of the incident.

The Investigation Section takes over the investigation upon receipt of the Spot Report.

The Investigation Section must prepare and submit a Progress Report within seventy-two (72) hours after receiving the Spot Report.

Copies of the Progress Report must be furnished to the Office of the Director of Corrections and the Office of the Superintendent of the New Bilibid Prison (NBP).

The final investigation report must be submitted within one (1) week after the submission of the Progress Report.

The final report should include the results of the investigation and the action taken relative to the case being investigated.

All cases handled must be kept in separate folders for ready reference.
