The issuance of OMVSP is based on Batas Pambansa Blg. 74, Republic Act No. 4136, and Administrative Order No. 009-2004 dated 03 September 2004.

Applications can be made at the LTO Central Office or any regional/district office which will forward the application to the OMVSP Committee Central Office for processing.

Required documents include a duly accomplished application form, certified true copy of OR/CR issued by the TDO, Plate Release Copy (for manually processed transactions using MVRR), certified true copy of official receipt of payment for the plates, and certification by the TDO that no plates have been previously issued.

The prices vary: Php 25,000 for any triple alpha and non-triple alpha with two numeric characters (01-99); Php 15,000 for any triple alpha and non-triple alpha with numeric 100; and Php 15,000 for combinations such as triple-alpha, triple-numeric, and others specified in the guidelines. These prices include the handling charge by the authorized LTO courier but exclude registration fees.

The plates are delivered by special courier with two delivery attempts made. After two unsuccessful attempts, further delivery attempts are at the expense of the motor vehicle owner. Unclaimed plates must be retrieved personally or by an authorized representative at the LTO Central Office.

This memorandum circular took effect on 11 October 2004.
