To establish a uniform and integrated system of crime reporting so the government can obtain an accurate picture of the national crime situation and thereby guide law enforcement and lawmaking.
Chiefs of city, municipal and municipal district police forces; the Chief of Constabulary; the Director of the National Bureau of Investigation; the Chairman of the Anti-Smuggling Action Center; the Commissioner of Customs; the Commissioner of Immigration; and heads of other national law enforcement agencies.
It must indicate whether the crime was handled exclusively by the reporting agency, handled jointly with other agencies, or not recorded in any city, municipal, or municipal district police blotter.
The report shall be prepared in quadruplicate on NBI Form ST-1 (copy attached) or on other crime report forms that may be prescribed later.
Original to the Chief of Constabulary or his representative; duplicate to the Chairman of the Police Commission; triplicate to the Director, National Bureau of Investigation; and quadruplicate retained as file copy.

On or before the 15th of every month.

The first monthly crime report shall cover the period January 1–31, 1967.
Through consistent monthly reporting requirements, standardized forms (NBI Form ST-1 or later prescribed forms), specified contents (including handling and blotter status), and centralized consolidation and tabulation by the Chief of Constabulary.
It is the recipient of the consolidated results forwarded after the Chief of Constabulary tabulates the monthly reports, serving as part of the coordination mechanism for crime statistics.
