It sets the rules on how PVAO applications are assigned claim numbers, including how the docket books are opened and how each application is encoded into a standardized claim number format.
The Administrative Division and each Field Team must open docket books.
They submit their applications to the concerned teams according to the procedures prescribed in Memorandum Circular No. 90-1.
When it is more convenient, the applicant, Accredited Contact Officer, or PVAO Contact Officer may submit the application by mail or by hand direct to the Administrative Division, PVAO, Manila.
Each application is entered in the docket book and assigned a claim number.
It consists of thirteen (13) characters, which are letters and numerals.
AA-A00-00-000000 (with grouping of benefit type, docketing office, year, and sequence number).
They indicate the type of benefit being claimed.
AP: Additional Pensions for Dependents of Totally Disabled Veterans; BA: Burial Assistance; DP: Death Pension for Indigent Parents; DS: Disability Pension; DW: Death Pension for Surviving Spouse and Minor Orphans; EB: Educational Benefit; OV: Old Age Pension for Veteran; OW: Old Age Pension for Surviving Spouse.
They indicate the docketing office.
MCO: Manila Central Office, PVAO; FO1: Field Team Number 1, Vigan; FO2: Field Team Number 2, Bayombong; FO3: Field Team Number 3, Dagupan; FO5: Field Team Number 5, Legaspi; FO6: Field Team Number 6, Iloilo; F6A: Field Team Number 6A, Bacolod; F07: Field Team Number 7, Cebu; FO8: Field Team Number 8, Tacloban; F8A: Field Team Number 8A, Catbalogan; F10: Field Team Number 10, Cagayan de Oro; F11: Field Team Number 11, Davao.
They indicate the year the application is filed (e.g., 90 for 1990, 91 for 1991).
They indicate the sequence number of the application received in the particular docketing office, from 000001 to 999999.
Use the benefit code DP, then MCO, then year 91, then the assigned sequence number (000001–999999). Format: DP-MCO-91-000000.
Use EB for Educational Benefit, F07 for Cebu field team, 90 for 1990, and sequence number 000015. Format: EB-F07-90-000015.
