All medical certificates for driver's license applicants must now adhere to a standardized format, ensuring clarity and completeness in health assessments by accredited physicians, as mandated by the LTO.

Legibility and Clarity of Findings

  • Accredited physicians must write their findings and recommendations legibly.
  • Terms used should be comprehensible even to non-medical personnel to avoid ambiguity.

Required Medical Entries

  • The medical certificate must include complete and properly filled entries on:
    • Visual acuity
    • Hearing
    • General physique
    • Blood pressure
    • Height
    • Weight
    • Presence or absence of contagious diseases
  • Additional comments regarding any condition affecting the applicant's fitness to drive must be indicated.

Non-acceptance of Non-conforming Certificates

  • The Chief of LTO Licensing Centers and District Offices will reject medical certificates that:

Obligation of Accredited Physicians Regarding License Type

  • Accredited physicians are no longer required to ascertain or check the kind of driver's license the applicant is applying for.

Supersession of Conflicting Orders

  • All previous orders and memoranda conflicting with this memorandum are considered superseded.

Enforcement and Compliance

  • The memorandum calls for strict compliance and is enforced as of December 18, 2008, under the authority of the Assistant Secretary Alberto H. Suansing.
