Legal basis and superseded rules
- Administrative Order No. RIB-2007-012 dated 17 May 2007 governs revised supervision and control over accredited physicians.
- LTO Memorandum Circular No. AHS-2008-273 revises/standardizes the required format for medical certificates used in the driver’s license process.
- Any orders and memoranda in conflict with LTO Memorandum Circular No. AHS-2008-273 are superseded.
Uniform medical certificate format
- Every medical certificate issued by any government or accredited physician must be in the prescribed form.
- Every medical certificate must be printed or prepared using white bond paper.
- The prescribed form is adopted by reference “as hereto attached,” establishing the standard layout that must be followed.
Requirements for accredited physicians
- The accredited physician must write findings and recommendations legibly.
- The accredited physician must use terms that can be understood even by non-medical personnel.
- All medical entries must be properly filled up, including: visual acuity, hearing, general physique, blood pressure, height, weight, and the presence or absence of contagious diseases.
- The accredited physician must include comments/remarks on other applicant conditions affecting fitness to drive.
Content completeness and legibility standards
- Medical certificates must include properly completed entries for the required categories, including the presence or absence of contagious diseases.
- Medical certificates must contain recommendations and findings that are written legibly and expressed in understandable terms for non-medical personnel.
- Comments on other driving-relevant conditions must be indicated in the certificate.
LTO acceptance and filing control
- The Chief of LTO Licensing Centers and District Offices must not accept medical certificates that do not conform to the prescribed form.
- The Chief of LTO Licensing Centers and District Offices must not accept medical certificates that are not properly and completely filled up with the required entries.
Driver’s license type requirement removed
- Accredited physicians are no longer required to check the kind of driver’s license being applied for.
