Required documents before COP processing
- Seafarers requesting issuance or revalidation of Certificates of Proficiency (COP) must submit a valid medical certificate before acceptance and processing of applications.
- The medical certificate must come from a DOH-accredited medical clinic.
- The medical certificate must have a validity period of at least one (1) year.
- Applications for issuance or revalidation of COPs must be accompanied by 2 pieces of passport size photos.
Photo uniform and rank requirements
- The 2 passport size photos must wear the appropriate uniform corresponding to the rank of the concerned seafarer.
- Seafarers not belonging to the Deck and or Engine department on board passenger ships must have their pictures taken wearing a white polo shirt with collar or white blouse with collar.
- Photos for the specified passenger-ship category must be taken on a white background.
Bulk submission by maritime training institutions
- Maritime Training Institutions (MTIs) or entities submitting requests for issuance of COP and other STCW-related certificates in bulk must submit scanned pictures.
- Bulk submissions must be provided in a compact or flush disk for facility in editing of the photos.
Practical compliance timing
- Compliance with the advisory’s photo and medical certificate requirements must occur prior to acceptance and processing of COP applications.
- The advisory’s requirements apply to applications for issuance/revalidation of COPs, including processing of STCW-related certificates submitted in bulk by MTIs or entities.
