Mandatory Fee Schedule and Compliance
- The prescribed fees represent the minimum rates to be charged and must not be undercut.
- Offering rebates or charging less than the prescribed rates constitutes a violation of Section 25.1 of the governing rules and regulations.
- Violations will be dealt with strictly to ensure the quality of medical examinations is maintained.
Basic Pre-Employment Medical Examination Fee and Inclusions
- The fixed cost for the basic pre-employment medical examination is set at P250.00.
- The basic examination comprises the following tests:
- Complete physical examination
- Chest X-ray (using plates not smaller than 11" x 14")
- Complete blood count (CBC) including hemoglobin determination
- Blood typing (ABO)
- Urinalysis
- Stool examination
- Psychometric evaluation
Additional Examinations for Seafarers
- Besides the basic examination, seafarers are required to undergo additional tests such as:
- Ishihara tests for color perception
- Audiometry
- These additional tests have separate charges which must be collected over and above the basic examination fee.

- Special examinations must be billed separately according to the following rates:

- Audiogram: P80.00
- Electrocardiogram (ECG): P120.00
- Fasting Blood Sugar (FBS): P70.00

- Pregnancy Test (urine): P60.00

- Erythrocyte Sedimentation: P30.00
- Malarial Smear: P40.00
- Sputum Examination for Acid-Fast Bacilli (AFB): P60.00
- Gram Stain: P40.00
- Smear Tests:
- Pap Smear (Cytology): P100.00
- Urethral Gram Stain: P40.00
- HIV/AIDS Test: P400.00
- Hepa-B Surface Antigen Test: P100.00

Effective Date and Enforcement
- The prescribed schedule of fees took effect immediately as of the date of filing, September 20, 1991.
- All accredited medical facilities must strictly comply with these regulations to ensure uniformity and quality in the medical screening of overseas Filipino workers and seafarers.
Legal Authority and Responsibility
- The memorandum circular is issued under authority of the Department of Health, specifically through the Bureau of Licensing and Regulation.
- Enforcement responsibilities include adherence by medical clinics and hospitals and regulatory oversight to penalize violations.
- Officials signing the order include the Director, the Undersecretary of Health for Standards and Regulation, and the Administrator of the responsible agency.
