- CSC Resolution No. 97-0398, dated January 17, 1997, formally adopts the newly revised Civil Service Form 212 or PDS.
- The revised form is mandated for use in all government agencies and offices, effective February 1, 1997.
- The adoption was promulgated through Memorandum Circular No. 6, issued on January 28, 1997.

- The revised form applies to all government personnel required to complete a Personal Data Sheet.
- It remains the official information sheet for government employees and a vital document for appointments.

- The Circular takes effect immediately upon issuance.
