Logbook Maintenance and Monitoring
- The Operations Division, Regional Office is responsible for maintaining a logbook to record all stamped chassis numbers.
- This logbook serves to monitor and prevent duplication of serial numbers, ensuring each chassis number is unique.
Reporting Requirements to Central Office
- A comprehensive report and endorsement of the stamped chassis numbers must be submitted to the Operations Division, Central Office.
- This measure is intended to maintain centralized records and oversight on all locally-built chassis and trailers.
Specifications and Format of Chassis Numbers
- The chassis number must contain a minimum of fourteen (14) characters and a maximum of seventeen (17) characters.
- The format example is: ABC-2015-0001-C, where:
- "ABC" represents the manufacturer's corporate code (e.g., Alpha Bravo Corporation),
- "2015" is the year of manufacture,
- "0001" is a sequential serial number, and
- "C" indicates the unit is a chassis.
- All characters must be legible, durable, and resistant to alteration to ensure authenticity and traceability.
Certification of Stock Reporting
- The Operations Division, Central Office issues a Certificate of Stock Reported to the manufacturer.
- This certificate certifies that the chassis or trailer has been duly reported to the Land Transportation Office (LTO).
Required Documents from Manufacturers
- Manufacturers must submit the following documents to validate the reporting process:
- Sales invoices of all materials used in the fabrication of the chassis,
- An affidavit of fabrication executed by the manufacturer,
- A stencil showing the chassis number used for stamping or engraving.
Supersession and Compliance
- All prior orders, memoranda, or circulars inconsistent with these guidelines are superseded.
- Strict compliance with these revised guidelines is mandated for all concerned parties to ensure uniformity and legality in the stamping and reporting process.
