PhilHealth Circular No. 006, issued by Dr. Eduardo P. Revised form: RF-1 box No. 8

  • PhilHealth Circular No. 006, S. 2012 revises the Employer Remittance Report (RF-1) by adding box No. 8.
  • Box No. 8 is the Date of Birth and Gender column.
  • The Date of Birth and Gender column is intended to facilitate the immediate registration and generation of PhilHealth Identification Number (PIN) of unregistered but actively paying employed-members.

Mandatory completion by employers

  • All employers must fill-out and supply the necessary information in box No. 8 of the revised RF-1 when the employee/s being reported have not been issued with permanent PINs.
  • The information supplied in box No. 8 serves as the basis for the registration, the generation of PINs, and the subsequent issuance of PhilHealth Identification Cards.
  • Employers must use the revised RF-1 in reporting monthly remittances covered by the circular’s effective period.

Updating member data and dependents

  • For updating a member’s data record and/or declaring dependents, the employer must still require employees to submit a properly accomplished PhilHealth Membership Registration Form (PMRF).
  • The PMRF submission must include applicable supporting documents.
  • The employer must forward the PMRF to PhilHealth Regional/Branch Offices together with the report of Employee-Members (ER2).

When the revised RF-1 applies

  • The revised RF-1 is used for submitting the monthly remittance report applicable for premium remittances of April 2012 onwards.
  • The requirement applies to both Hard Copy and Soft Copy formats.

Relationship to prior PhilHealth rules

  • PhilHealth Circular No. 13, s. 2010 remains in full force and effect for all other provisions not inconsistent with PhilHealth Circular No. 006, S. 2012.

Effectivity and publication rule

  • PhilHealth Circular No. 006, S. 2012 takes effect fifteen (15) days upon publication in a newspaper of general circulation.
