  • Statement describing any anticipated risks, problems, or complications associated with the request.

Fund Availability

  • Certification from the agency’s budget or finance officer verifying the availability of funds if the issuance or approval requires funding.

Draft Action Document

  • Inclusion of a draft of the dispositive action document to be issued by the OP.
  • Subject to adjustment and finalization by the OP.

Consequences of Non-Compliance

  • Requests failing to comply with these requirements may be returned to the requesting entity for completion of necessary staff work.
