Permit Requirement and Application Procedure

  • Collection at remote facilities requires a Permit for Remote Collection of Drug Testing Specimen (PRC-DTS).
  • The permit must be secured from the DOH-Bureau of Health Facilities and Services (BHFS) or the Community Health Division (CHD).
  • Application must be made at least ten (10) working days before the collection date.

Documentary Requirements for Permit Application

  • Letter of request to conduct remote collection.
  • Notarized Memorandum of Agreement or contract between involved parties.
  • List of authorized personnel designated for remote collection.
  • Operational procedures covering collection, transport, and handling.
  • List of supplies and transport materials such as ice chests.

Fee and Validity of Permit

  • Laboratories must pay a fee of Php 2,500 per remote collection site.
  • The PRC-DTS is valid only on the specified date indicated on the permit.

Location and Accessibility of Collection Site

  • The remote collection site must be within the same administrative region as the laboratory.
  • Site must be accessible by land travel within a maximum of two (2) hours.

Posting and Inspection Requirements

  • The permit must be posted conspicuously at both the laboratory and the remote collection site.
  • DOH-BHFS reserves the right to inspect the facility before issuing the permit.
  • The collection site must have secure and suitable areas for specimen collection, storage, supplies, and records.

Monitoring and Operational Restrictions

  • The facility is subject to monitoring during specimen collection.
  • No drug testing examinations may be conducted at the remote collection site.

Minimum Client Volume

  • Remote collection sites must have a minimum of fifty (50) clients or donors.

Record Keeping and Reporting

  • Laboratories must submit a copy of the completed remote collection record as per the assessment tool (Annex A).

Rescheduling and Notification

  • Failure to collect specimens on the scheduled date requires the laboratory to notify DOH-BHFS/CHD in writing at least one day prior.
  • Rescheduling is at the discretion of DOH-BHFS/CHD.

Penalties and Compliance

  • Violations of these guidelines, the Implementing Rules and Regulations (IRR) for DTL accreditation, or Manual of Operations (MANOPS) may result in suspension or revocation of the PRC-DTS and laboratory accreditation.
