  • For the first year of policy implementation, members/new enrollees whose professions are included in Annex C and whose monthly family income in the last 12 months is P25,000 and below must pay PHP 300 per quarter or PHP 1,200 per year.
  • For succeeding years, members whose professions are included in Annex C must pay PHP 900 per quarter or PHP 3,600 per year.
  • For succeeding years, members whose professions are included in Annex C and whose average monthly family income in the last 12 months is P25,000 and below must continue paying PHP 300 per quarter or PHP 1,200 per year.
  • For members/new enrollees whose profession is not included in Annex C, the premium contribution is PHP 300 per quarter or PHP 1,200 per year.

Registration venue and implementation mechanics

  • All existing IPMs and new enrollees whose professions are included under the List of Professionals/Other Professionals (Annex C) must register at the nearest Service Office.
  • The registration and documentation process is carried out through submission of the PMRF with profession and income-reflecting attachments, including PRC ID, Business Permit, and (when applicable) ITR.
