Objective
- Facilitate the issuance of STCW '95 certificates to successful examinees of the marine deck officers licensure exams.
Purpose
- Provide clear guidelines for applicants who have completed approved training courses necessary to obtain STCW '95 certificates.
- Ensure STCW certificates are issued during or immediately after the oath-taking ceremony.
- Eliminate the need for successful examinees to separately apply for STCW certificates post-examination.
Scope and Applicability
- Applies to all marine deck officers licensure examination applicants who have completed or taken required STCW training courses.
- Requires applicants to be medically fit with good eyesight and hearing, validated through a medical certificate issued by DOH-accredited facilities per STCW Code standards.
- Training courses must be completed at Maritime Training Council (MTC) accredited centers and recognized by the Board if required.
- Each Marine Deck Officer is assigned a unique permanent registration number for all official certificates.
- Excludes officers serving on warships, naval auxiliaries, state non-commercial ships, fishing vessels, pleasure yachts not in commercial trade, and wooden ships of primitive build.
Documentary Requirements
For First-Time Applicants:
- Accomplished Application Forms (PRC-MDO Form No. 3 and PRC-STCW Form No. 18).
- Original and photocopies of:
- Current Certificate of Registration and PRC ID.
- Ship Simulator and Bridge Teamwork Certificate and Management Level Course proof.
- Transcript of Records with required institutional details and remarks for licensure.
- NBI Clearance valid through exam date.
- Comprehensive Medical Certificate valid at least one month post exam.
- Birth Certificate in Security Paper (SECPA).
- Current Community Tax Certificate/Cedula.
- Training Record Book (TRB).
- Required seagoing service documentation depending on rank (e.g., 12 months as Officer in Charge, 24 months for Chief Mate, 12 months sea service as Chief Mate for Master Mariner).
- Sea service proofs including Seafarers' Identification and Record Book (SIRB), company certification, Certificate of Sea Service, and POEA contracts or CREWLISTs.
- Certificates for specific training courses and telecommunications certification.
- Four passport-size photographs meeting specific criteria.
- Documentary stamp, mailing envelope with metered stamp, and examination fee.
For Repeaters or Conditioned Applicants:
- Similar document requirements but streamlined forms and no sea service proof required anew.
Procedural Steps
- Secure and fill out application and STCW forms from the Marine Deck Officers Division (MDOD) Information Counter.
- Submit forms and documentary requirements to the Application Processing Counter for evaluation.
- Verification of document authenticity; questionable documents are subjected to further legal review.
- Log applicant details (name, rank, and application status) for record.
- Upon authorization, proceed for payment of fees and processing of payment receipts, Notice of Admission, and schedule issuance.
- Verification of completeness of submitted documents and issuance of examination permits.
- Pre-examination notification of examination site details.
- STCW Records Verifier confirms training certificate authenticity through MTC reports.
- Applications found compliant are forwarded for assessment and approval by Qualified Assessor/Board member.
- Approved applications are passed to Application Section Head for reporting.
- Status reports are submitted to the Chief of the Examination Division.
- Successful candidates must pay fees for registration, PRC ID, and STCW certificates.
Registration and Identification
- A unique permanent registration/certificate number assigned to each Marine Deck Officer.
- This unique number is reflected on all official credentials (Certificate of Registration, Certificate of Competency, Endorsement Certificate).
Exclusionary Provisions
- Officers serving on government non-commercial ships (warships, naval auxiliaries).
- Crews on fishing vessels, pleasure yachts not in commercial trade, and wooden ships of primitive construction.
Medical Requirements
- Applicants must provide a comprehensive medical certificate proving fitness, particularly eyesight and hearing, issued by DOH-accredited medical institutions.
- The certificate must be valid for at least one month after the examination date.
Enforcement and Verification
- Fraudulent or suspicious documents trigger a referral for legal action through a formal process involving the division chief and Marine Deck Officers legal counsel.
Financial Requirements
- Applicants must pay prescribed examination fees, documentary stamps, and costs related to registration and certification.
Effectivity
- The memorandum circular supersedes previous issuances to the contrary.
- It took effect immediately upon adoption on June 18, 2008.
