  • Applications not acted upon by the LEB within the 15-day period are presumed approved after the lapse of the period.
  • Presumed approval does not prevent the LEB from directing adjustments or corrections.
  • Applications must be filed in triplicate using the form prescribed by the Chairman of the LEB.

  • The fine for late filing is computed per day of delay under the stated PHP 100 per day rule.
