Submission Requirements for Manning Agencies and Shipping Companies
- Request must be submitted to the Manpower Registry Division - Employment Branch of the Welfare and Employment Office.
- Must include a letter-request signed by the company president or authorized signatory.
- Required supporting documents include:
- Seafarer's Seamen's Service Record Book (SSRB) and original SRC.
- Completed and signed Seafarer’s Information Sheet with thumbmark and a 1 x 1 photograph.
- Original Professional Regulation Commission (PRC) ID license for marine officers.
- Original First Class Radiotelegraphy Certificate for Radio Operators.
- Original Radar Observer's course and Radar Simulator Certificates for deck officers.
- Additional documents as may be required.
