Procedural Requirements for Deputized Agents
- Upon receiving court referrals, agents must prepare an index card recording patient's personal info, progress notes, and any violations.
- Periodic home visits are mandatory, with outcomes documented on the index card.
- Any violations by patients or guardians must be reported promptly to the court and DDB.
- At treatment completion, agents submit a detailed report with recommendations on rehabilitation status to the DDB.
- The DDB evaluates reports and may require pre-release evaluation sessions with patient and agent personnel.
- If further treatment is needed, agents must notify the DDB, which will report to the court for necessary action.
Terms and Conditions for Aftercare and Follow-up Treatment
- Patients must report at least twice monthly to designated agents for guidance, counseling, and other services.
- Patients must remain at home from 8:00 PM to 5:00 AM unless accompanied by approved relatives; adjustments allowed for employment or schooling.
- Patients cannot change residence without prior knowledge and approval by the DDB or its agents.
- Patients must maintain good conduct, avoid disreputable persons and places, and limit alcohol consumption.
- Parents or guardians are required to submit reports using a specified DDB form.
Effectivity of the Regulation
- The regulation is effective immediately upon adoption on November 17, 1982.
