Scope and Basis

Applications for Duplicate/Triplicate Plates

  • Must be filed at the LTO District Office that issued the original plates, except when owner/operator has moved far from original office.
  • Verification with original issuing office is mandatory when application is filed elsewhere.
  • Required documents include notarized affidavit of loss, certified true copies of OR/CR with originals for authentication, MV inspection report, and clearances (MMDA, LES or Operations Division).
  • If one plate lost, the other must be surrendered and destroyed; if both lost, affidavit must state so with inspector certification.
  • Online requisition procedures and timelines must be followed.
  • Limit of two duplicate plate applications per vehicle; third application requires investigation and resolution approval before processing.

Prohibition on New Plate Issuance for Previously Assigned Numbers

  • Per BP 43, plates assigned to a vehicle are permanent and not to be changed even if vehicle recovered after theft or involved in crime.
  • Only brand new vehicles may apply for Optional Motor Vehicle Special Plates (OMVSP); used/imported vehicles excluded.
  • Special plate series must be encoded and deducted from inventory prior to production to avoid duplication.
  • Only original registering office may issue new plates; unauthorized issuance subject to administrative penalties.

Change of Vehicle Classification

  • Change from private to for-hire classification must be filed at LTO office where CPC by LTFRB is issued.
  • Original plates retained; for-hire sticker issued instead.
  • Change classification fee of P150 applied in addition to regular fees.

Encoding and Uploading Procedures

  • Plates (new, duplicate, special, etc.) must be uploaded in the Information Management System within 24 hours of release.
  • In cases of duplicate issuance, vehicle with earlier plate retains it; other vehicle must surrender plates prior to new issuance.
  • Clear procedural flow for plate printing and issuance must be in place to identify responsible personnel and record all transactions.
  • Careful data entry required to avoid encoding errors.
  • No office may encode plate numbers outside their allocated series or override existing ones.

Application for Duplicate Windshield Stickers/Year Tags

  • Applications must be filed at original issuing office only.
  • Same documentary requirements as duplicate plates apply.
  • Only one duplicate application allowed; second application denied to prevent misuse.
  • Duplicate stickers issued are not new sets but accompanied by affidavit of loss with serial number, which must be shown during enforcement checks.
  • Prescribed fees apply.

Issuance of Replacement Plates for Dilapidated/Obscure Plates

  • Requirements include certified copies of OR/CR, affidavit of dilapidation, deed of sale if buyer executes affidavit, corporate resolutions if applicable, and payment receipt.

Penalties

  • Violations of guidelines by personnel subject to administrative actions under Civil Service Rules.

Effectivity

  • Guidelines effective 15 days after publication and submission to National Administrative Register.
