Law Summary
Mandatory Enrollment and Timing
- Retiring employees from both government and private sectors who meet eligibility are to be enrolled automatically in the Lifetime Member Program (LMP).
- Employers must facilitate this enrollment at least three months prior to the employee's retirement.
Application Process
- Application for LMP is filed with PhilHealth immediately after the employee's retirement application approval.
- Employer’s HR Office or authorized representative is responsible for submission.
Required Documents for Processing
- Two copies of the duly accomplished PhilHealth Member Registration Form (PMRF).
- Two recent 1x1 photographs of the member.
- Certified copies of:
- Birth certificate or alternative documents as specified.
- Approved retirement application.
- For private sector: Employer certification confirming 10 years continuous employment and remittance of PhilHealth contributions with notes on leave without pay.
- For government sector: Signed Service Record indicating credited service and leave without pay, plus certification of premium contributions with official receipt numbers.
Computation of Contributions
- Contributions from previous employers or other PhilHealth program categories count towards the required 120 months.
- Submission of payment proofs or certification with official receipt numbers is mandatory.
Special Cases
- Retiring members without 120 contributions but who reached retirement age can still be enrolled if proof of payments from other sources exist.
- Members who completed 120 contributions but have not reached retirement age are advised to continue as Individually Paying Members until eligible for LMP.
Submission and Issuance
- Application documents must be submitted to the nearest PhilHealth Regional, Service, or Branch Office.
- Upon validation, PhilHealth issues the Lifetime Member ID card and Member Data Record (MDR).
- Employers’ representatives must ensure release of these documents before the last day of employment.
Effectivity
- This Circular takes effect fifteen days after publication in a newspaper of general circulation.
