Types of Reports and Documents for Regional Submission
- Monthly Materials Reports (MMRs) including:
- Summary of field tests performed
- Status of tests
- Works Sheets for all field tests
- Test Reports on concrete samples
- Inspection Reports/Certifications related to quality control
- Work Sheets for concrete and asphalt mix designs with on-site adjustments
- Materials Quality Control Progress Report
- Contract documents including a copy of the contract, plans, and specifications (to be submitted before project commencement)
- Change or Extra Work Orders affecting testing requirements (to be submitted upon approval)
Exceptions for PMO and RPMO Projects
- Projects implemented by Project Management Offices (PMOs) and Regional PMOs (RPMOs) must still submit all quality control reports/documents to the BRS.
- These projects generally involve:
- Multiple regular or special work items
- Use of new technologies or products
- Large contract amounts (in millions of pesos)
- Long contract durations (one year or more)
Role and Responsibilities of Quality Assurance Units (QAUs)
- QAUs are assigned to review and evaluate the accuracy, truthfulness, and timeliness of the submitted quality control reports and documents.
- The evaluation results affect the Annual Rating of Quality Control concerning project implementation for the Regional and District Offices.
Record-Keeping and Availability of Documents
- Regional Offices must maintain complete records or copies of the required documents.
- Documents should be specifically available at the Materials Quality Control and Hydrology Division (MQCHD) for ease of access during quarterly QAU inspections and assessments.
Monthly Reporting to the Bureau of Research and Standards
- Regional Offices are required to submit a prescribed Monitoring of Quality Assurance Implementation Form to the BRS during the first week of each month.
- This report must include essential data derived from project documents as mandated by Department policies.
Supersession and Effectivity
- This Department Order supersedes Department Order No. 55, series of 1989.
- The Order took effect immediately upon its adoption on March 26, 2010.
