Policy linkage and purpose
- PhilHealth Circular No. 03, s. 2011 issues amendments in line with PhilHealth Circular No. 25, s. 2010, known as the Nine (9) Months Contribution as a Requirement for Benefit Availment.
What is amended in Claim Form I
- Item number four (4) of the Employer’s Certification (Annex A) under Claim Form I, Part II is amended.
- The amended certification requires certification that all monthly premium contributions for and on behalf of the member while employed in the company have been deducted/collected and remitted to PhilHealth.
- The amended certification expressly includes the applicable nine (9) monthly premium contributions within the past twelve (12) months period prior to the first day of this confinement.
- The amended certification further certifies that the information supplied by the member and/or the member’s representative on Part I is consistent with available records.
What is amended in implementing guidelines
- The certification of employer portion (Annex B) under the Implementing Guidelines on the Proper Accomplishment of Revised PhilHealth Claim I is amended.
- The amended employer certification requires the employer of the authorized representative to affix a signature certifying that all monthly premium contributions for and on behalf of the member while employed in the employer’s company have been deducted/collected and remitted to PhilHealth.
- The amended employer certification expressly includes the applicable nine (9) monthly premium contributions within the past twelve (12) month period prior to the first day of confinement.
- The amended employer certification further requires certification that the information supplied by the member or the member’s representative is consistent with the employer’s available records.
Date coverage and transitory rule
- The amendments under PhilHealth Circular No. 03, s. 2011 apply to all claims with admission dates starting July 1, 2011.
- Provisions in prior issuances that are not inconsistent with PhilHealth Circular No. 03, s. 2011 remain in full force and effect.
