Registration of Births: Procedures and Responsibilities
- Birth registration must be done by the attending physician, nurse, midwife, hilot, hospital or clinic administrator, or in their absence, by the parents or any responsible family member or knowledgeable person.

Registration of Deaths: Procedures and Responsibilities
- Deaths must be reported within 48 hours to the Local Health Officer by the nearest responsible relative or any knowledgeable person.
- The Local Health Officer issues the death certificate and orders its registration within thirty (30) days from the date of death, without fees or fines.
- If a physician attended to the deceased, the physician must issue the death certificate within 48 hours.
- Joint liability applies to the attending physician and responsible family member or knowledgeable person if the death is not reported.
- If no physician attended or the death did not occur in a hospital, liability for failure to report rests solely on the responsible family member.
Authority to Extend Registration Period
- The Director-General of the National Economic and Development Authority (NEDA) may extend the registration period for unregistered births and deaths in meritorious cases.
- Extension can be made upon recommendation of the Civil Registrar-General.
Repeal and Effectivity
- Inconsistent laws or provisions are repealed, amended, or modified accordingly to conform with this Decree.
- The Decree took effect immediately upon issuance.
Legal Obligations and Penalties
- No fees or fines are imposed for late registrations within the extended periods.
- Failure to register births or report deaths within prescribed periods imposes joint or primary liability, depending on the circumstances and involved parties.
