Option b.1 requires the applicant to submit:
- Original and Photocopy of the Certificates of Live Birth printed on National Statistics Office (NSO) security paper; and
- The certificates must be signed by Ms. Carmelita N. Ericta; and
- Submission must include an attached original receipt.
Option b.2 requires the applicant to submit:
- Original and Photocopy of the computer generated NSO Certificates of Live Birth; and
- The certificates must state at the dorsal portion: “A Certified True Copy: Office of the Civil Registrar General”; and
- The certificates must be duly signed by an authorized NSO statistician; and
- The submission must include the certificates in security paper; and
- Submission must include an attached original receipt.
Implementation and dissemination
- PRC Resolution No. 2003-159A directs that copies of the resolution be furnished to all schools, colleges or universities, public and private, offering courses for licensure examinations.
- PRC Resolution No. 2003-159A directs that copies of the resolution be furnished to accredited professional organizations for their information and guidance.
- The resolution instructs dissemination “for their information and guidance” in relation to applicants’ document submission requirements.
Effectivity and transitory publication rule
