Amendment on Birth Certificate Requirements

PRC Resolution No. 2003-159A is anchored on PRC Resolution No. 2002-76, Series of 2002, issued November 29, 2002.
PRC Resolution No. 2002-76 provides the “Revised Guidelines and Procedures in Filing of Applications for Licensure Examinations.”
PRC Resolution No. 2003-159A specifically amends Item II (b) of PRC Resolution No. 2002-76 regarding required birth certificate documents.
PRC Resolution No. 2003-159A recognizes a Memorandum of Agreement entered into by the Commission with the National Statistics Office (NSO) to assist PRC clients applying for licensure examinations.

PRC Resolution No. 2003-159A is adopted to address the submission of spurious or fraudulent Certificates of Live Birth issued by Local Civil Registrars.
PRC Resolution No. 2003-159A is intended to further establish the authenticity of Birth Certificates submitted by applicants and registered professionals.
PRC Resolution No. 2003-159A takes into account the NSO’s objective of providing faster and more reliable service to the public at large.

PRC Resolution No. 2003-159A applies to applicants in licensure examinations submitting Certificates of Live Birth as part of their application documents.
PRC Resolution No. 2003-159A amends the specific document requirement previously stated under Item II (b) of PRC Resolution No. 2002-76.
The amendment is directed toward birth certificate authenticity for individuals seeking licensure under PRC processes.

PRC Resolution No. 2003-159A replaces Item II (b) with the following amended birth certificate submission alternatives:
Option b.1 requires the applicant to submit:
- Original and Photocopy of the Certificates of Live Birth printed on National Statistics Office (NSO) security paper; and
- The certificates must be signed by Ms. Carmelita N. Ericta; and
- Submission must include an attached original receipt.
Option b.2 requires the applicant to submit:
- Original and Photocopy of the computer generated NSO Certificates of Live Birth; and
- The certificates must state at the dorsal portion: “A Certified True Copy: Office of the Civil Registrar General”; and
- The certificates must be duly signed by an authorized NSO statistician; and
- The submission must include the certificates in security paper; and
- Submission must include an attached original receipt.

PRC Resolution No. 2003-159A directs that copies of the resolution be furnished to all schools, colleges or universities, public and private, offering courses for licensure examinations.
PRC Resolution No. 2003-159A directs that copies of the resolution be furnished to accredited professional organizations for their information and guidance.
The resolution instructs dissemination “for their information and guidance” in relation to applicants’ document submission requirements.

PRC Resolution No. 2003-159A takes effect upon approval and completion of publication in Official Gazette or in a newspaper of general circulation in the Philippines, whichever is earlier.
