Procedures for Issuance of ID Sheets
- ID sheets are given upon presentation of approved Job Orders at Windows 1 and 2 of the ECPB on the 2nd floor of the POEA Building.
- Inquiries can be made at the same locations.
Guidelines on Accomplishing the ID Sheets
OCW Information
- Entries must be in block letters.
- For OCWs with multiple middle names, the commonly used middle initial matching official documents must be used.
- Date of birth must be accurately indicated, with special attention to the year to avoid errors like writing the current year instead of birth year.
OCW Photo Specifications
- Only 2x2 inch passport-quality black and white photos with white background are accepted.
- Photos must be trimmed to fit within the designated yellow box on the sheet.
- The face must be centered with the top of the head about 1/8" to 1/4" from the upper edge of the yellow box.
- Facial features must be clearly visible.
- Photos must be pasted securely; stapling is not allowed.
OCW Signature Requirements
- The OCW must sign inside the yellow signature box.
- Only the OCW's signature is allowed in the signature box, not printed names.
- Medium thickness black sign pen must be used; no other colors allowed.
Additional Handling Instructions
- Neither the photo nor the ID sheet should be folded.
- Samples of correctly accomplished ID sheets are displayed at designated receiving windows for guidance.
Submission Procedures
- Properly filled out ID sheets must be attached to the corresponding Request for Processing (RFP) forms and submitted at the ECPB.
- Incorrectly filled ID sheets will be returned for correction and can be resubmitted the following day at Windows 1 and 2.
Releasing Timelines for OCW ID Cards
- One-Stop Shop and Land-based workers: Available 5 working days after submission; claim at Windows 1 and 2 of ECPB.
- Sea-based workers: Available 3 working days after submission; claim at same windows.
- GPB Name hires: Available 1 working day after submission; claim at GPB windows.
Effective Date and Administration
- The guidelines are effective immediately upon adoption.
