Guidelines on Accomplishing the ID Sheets

OCW Information

  • Entries must be in block letters.
  • For OCWs with multiple middle names, the commonly used middle initial matching official documents must be used.
  • Date of birth must be accurately indicated, with special attention to the year to avoid errors like writing the current year instead of birth year.

OCW Photo Specifications

  • Only 2x2 inch passport-quality black and white photos with white background are accepted.
  • Photos must be trimmed to fit within the designated yellow box on the sheet.
  • The face must be centered with the top of the head about 1/8" to 1/4" from the upper edge of the yellow box.
  • Facial features must be clearly visible.
  • Photos must be pasted securely; stapling is not allowed.

OCW Signature Requirements

  • The OCW must sign inside the yellow signature box.
  • Only the OCW's signature is allowed in the signature box, not printed names.
  • Medium thickness black sign pen must be used; no other colors allowed.

Additional Handling Instructions

  • Neither the photo nor the ID sheet should be folded.
  • Samples of correctly accomplished ID sheets are displayed at designated receiving windows for guidance.

Submission Procedures

  • Properly filled out ID sheets must be attached to the corresponding Request for Processing (RFP) forms and submitted at the ECPB.
  • Incorrectly filled ID sheets will be returned for correction and can be resubmitted the following day at Windows 1 and 2.

Releasing Timelines for OCW ID Cards

  • One-Stop Shop and Land-based workers: Available 5 working days after submission; claim at Windows 1 and 2 of ECPB.
