Office Order No. 04-09 provides procedural guidelines for the re-registration of cooperatives in the Philippines, ensuring compliance with the Cooperative Code of 2008 and maintaining their legal status.

It covers step-by-step procedures in the re-registration of cooperatives to comply with RA 9520 and related MC issued for that purpose.

From March 22, 2009 to March 23, 2010.

All registered and confirmed cooperatives with the CDA regardless of type and category, provided they comply with RA 9520; cooperatives that have been cancelled or ordered dissolved are disqualified.

At the concerned CDA Extension Office (EO) where the cooperative was originally registered.

At the CDA Central Office.

In the CDA Extension Office that has jurisdiction over its present principal office address.

1) Photocopy of certificate of registration/confirmation (and certificate of amendments if applicable); 2) Photocopy of registered articles of cooperation (and amendments); 3) Photocopy of registered by-laws (and amendments); 4) Original copy of the latest (2008) audited financial statements; and 5) Re-registration fee of PHP 200.00.

The cooperative must have a 10-digit CIN issued by the CDA to access and use the web-based re-registration system.

It must fill up the application form online and secure the notification with checklist of requirements; then it must file the required documents at the concerned CDA office where it was registered.

An online notification is displayed directing the applicant to proceed to the concerned CDA office for further evaluation of its status.

Name of cooperative; BIR TIN (if available); contact person; designation; mobile number; telephone number; street address (number, street, barangay); email address; total assets as of December 2008; sub-category/type; province/district/city-municipality (selected via dropdowns).

The applicant clicks Submit; a pop-up appears containing the notification/instructions for next actions, and the message should be printed for reference.

Sr. CDS signs in and validates entries; if no problems it submits; if problems it informs the cooperative and proceeds to next record. Supvg. CDS then reviews and submits if in order and generates a new registration number; if problems it informs Sr. CDS.

Sr. CDS checks re-registered cooperatives and prints three copies of the new CoR using the EO database template; prepares an order of payment attached to the certificate; forwards for initials (Sr. CDS → Supvg. CDS → ACD). ACD initials copies and returns them to the registration unit. Registration releasing staff directs payment to the cashier, records official receipt details on the duplicate certificate, stamps the dry seal, and releases the certificate after the applicant signs.

Original to the cooperative; duplicate to the CDA Extension Office; triplicate to CDA Central Office.

Sr. CDS verifies the specific online instruction/reason for non-operating status, evaluates the reason, and formally advises the applicant to comply to justify reclassification into an operating cooperative. The applicant must submit proof of operation satisfactory to the CDA unit, subject to prior written approval of the ACD; acceptable documents may include audited financial statement as of 2008, minutes of the general assembly meeting, and a copy of Certificate of Good Standing for 2007 or 2008.
