Effective January 1, 2004, the new premium contribution schedule for the employed and sponsored (indigent) sectors revises the minimum and maximum salary bases and introduces updated contribution rates to ensure the actuarial viability of the Health Insurance Fund and the provision of new benefits.

For the Employed Sector, the revised contribution schedule took effect starting January 1, 2004 (with further changes effective January 1, 2005).

The minimum SB was revised from ₱3,000 to ₱4,000, and the maximum SB was revised from ₱10,000 to ₱15,000.

Failure to deduct and/or consequently remit said contributions shall make the employer liable under the penal provisions provided in Article X of RA 7875 (National Health Insurance Act of 1995).

Employers are enjoined to submit the Quarterly Remittance Reports textfile format on or before the 15th day of the month following the applicable quarter.
