The Certificate of Eligibility or PHIC Form CE1 is issued to PhilHealth indigent members and their dependents to ensure accessibility to hospitalization benefits and serves as a substitute document in the absence of required membership and/or dependency documents.

The LGU should submit to PhilHealth Regional Office the list of members for renewal at least one month prior to the expiration for encoding, ID generation, and distribution.

Lost PhilHealth ID cards shall be replaced and released immediately.

This Circular took effect 15 days after publication in two widely circulated daily newspapers.

This Circular superseded PhilHealth Circular No. 20, series of 2001 on PHIC Form CE1.
