An applicant can file at the CHR Regional Office concerned with territorial jurisdiction over the applicant's official station or assignment, or at the General Records and Archives Division, Office of the Commission Secretary, CHR Central Office.

The required fee is one hundred pesos (P100.00).

Applicants must submit two (2) passport size ID pictures taken within six (6) months prior to filing; computer scanned or xerox copies are not accepted.
