All banks must submit an IT Profile report detailing their technology systems and risk profiles as of December 31, 2011, to the Bangko Sentral ng Pilipinas by March 31, 2012, with annual updates required thereafter.

The subject must be: 'ITPROFILE,< bankname >,< reference period >' (example given: 'ITPROFILE < bankname >, 31 December 2011').

The certification must be duly notarized and signed by the authorized official of the reporting institution.

It must be sent via facsimile to the Director of the BSP Supervisory Data Center (SDC) at (632) 708-7554 or 708-7558.

The institution may submit the report in CD together with the aforementioned certification through messengerial or postal services within the prescribed deadline.
