DEPED Order No. 79 establishes a comprehensive assessment and evaluation system for public elementary and secondary schools, mandating that grades reflect true student performance through a balanced approach of traditional and non-traditional assessment methods, with specific guidelines on grading and competency requirements.

The lowest grade that shall appear on the report card is sixty-five percent (65%). Any true grade below 65% shall be retained in the class record but not shown in the report card.

Grades shall be recorded as raw scores, totaled at the end of each grading period, and then computed as percentages by dividing the student's score by the highest possible score and multiplying by 100.

Attainment of at least 75% of the competencies is required to pass each subject.

No, grades shall not be computed based on any transmutation table that equates zero to a pre-selected base like 50 or 70 and adjusts other scores accordingly.

Tests shall be designed with 60% easy items focused on basic content and skills, 30% medium-level items focused on higher-level skills, and 10% difficult items focused on desirable content or skills to distinguish fast learners.

Non-traditional assessments such as open-ended questions, performance-based assessments, and portfolio assessments are encouraged to complement traditional assessments.

Grades shall be computed based on percentage weights allocated for various components including quizzes, participation, projects, and periodical tests as defined in DepEd Order No. 80, s. 1993 for elementary and DepEd Order No. 37, s. 2003 for secondary levels.

The final grade is computed as the average of the four periodical grades in each subject.

Pertinent provisions and/or sections of DepEd Orders Nos. 80, s. 1993 and 66, s. 1995; and DepEd Orders Nos. 37 and 70, s. 2003 that are inconsistent with Order No. 79, s. 2003 are revoked or amended accordingly.

The guidelines were adopted to be effective starting the Second Grading Period of School Year 2003-2004.
