All medical certificates for driver's license applicants must now adhere to a standardized format, ensuring clarity and completeness in health assessments by accredited physicians, as mandated by the LTO.

Non-acceptance of Non-conforming Certificates

  • The Chief of LTO Licensing Centers and District Offices will reject medical certificates that:
    • Do not conform to the prescribed form
    • Are not properly or completely filled.

Obligation of Accredited Physicians Regarding License Type

  • Accredited physicians are no longer required to ascertain or check the kind of driver's license the applicant is applying for.

Supersession of Conflicting Orders

  • All previous orders and memoranda conflicting with this memorandum are considered superseded.

Enforcement and Compliance

  • The memorandum calls for strict compliance and is enforced as of December 18, 2008, under the authority of the Assistant Secretary Alberto H. Suansing.
